What personal information do we collect?
DNA MEDICAL CLINIC will collect, use and disclose your personal information, only with your consent (some of which may constitute personal health information), including your name, address, phone number, e-mail address, date of birth, gender, age, medical history, medication list, various health information and genetic information for the purposes of providing and managing the Services.
Your personal information and any other information we collect from or on you will not be used or disclosed by DNA MEDICAL CLINIC for any other purpose unless: (a) your express and informed consent is first obtained, (b) the use, collection or disclosure is permitted or required by law, or (c) the information is rendered anonymous and used by DNA MEDICAL CLINIC solely for scientific research, planning, statistical or publication purposes in the field of genetic testing.
We may collect personal information from you when you voluntarily provide it to us through a number of features and Services you may choose, purchase, or made available to you (for example, by accessing or using this Site). For example, we may ask you to provide personal information if you: register as a user of this Site; participate in a review, survey, a consumer research panel, or similar type of activity; ask to receive online information from us; purchase a Service; and/or otherwise participate in features of our Site that ask for personal information.
In addition, if you purchase or otherwise request a product, Service, publication, or promotional offering from us you may be required to provide your contact and/or payment information (such as a credit card number) in order for us to process your request.

Being able to access our cookies allows us to personalize this website and improve your experience at this website. How do we share personal information?
Except as described below, we do not sell, transfer or otherwise disclose to third parties the personal information we collect from you or on this Site. At times, we provide your personal information to joint promotion partners and third parties who provide services on our behalf. We may also use third-party service providers to assist us in aggregating and anonymizing your personal information, so that it does not include any information from which any individual can be identified. We do not consider such aggregated and anonymized information to be personal information. You hereby expressly consent to DNA MEDICAL CLINIC using anonymous information that it has collected from you to prepare reports and other publications containing anonymous information only for internal use and for disclosure to third parties (including government, pharmaceutical companies, physicians, nutritionists, pharmacists, drug manufacturers and others involved in health care or in the medical field).
When DNA MEDICAL CLINIC genetic research staff or third-party collaborators or service providers publish results (on their websites or in academic journals) from tests and Services conducted by us, your genetic and self-reported information may be included within pooled summaries that are made public. Identification of your individual-level data from those summaries would be extremely difficult, but it is possible that a third party that has obtained some of your genetic data could compare that partial data to the published results and infer some of your survey responses.

How We Protect Personal Information

A file containing your personal information will be kept at the offices of DNA MEDICAL CLINIC located at 273 Bruyère Street, Ottawa, Ontario or on its secure servers located in Canada. As stated above, your personal information may be transferred to third-party service providers (including laboratories, distributors, nutritionists, dieticians, doctors, pharmacists and other parties) who provide services on behalf of DNA MEDICAL CLINIC and who are also restricted from using or disclosing information for any purpose other than to provide the services they are contractually obligated to provide to DNA MEDICAL CLINIC. Some of these service providers may be located outside of Canada, including in the United States. Measures DNA MEDICAL CLINIC may use to protect your personal information while it is being processed by its third-party service providers are subject to the legal requirements of the foreign jurisdictions where your personal information is transferred, including lawful requirements to disclose personal information to government authorities in certain circumstances.
