The services offered by DNA MEDICAL CLINIC are sensitive in nature and require the collection, use and disclosure of your personal information. These products and services may include the sale of DNA MEDICAL CLINIC testing kits and the performance of various genetic, nutritional, pharmacological and other testing and services, including but not limited to the following: Cancer Plan, ADHD Plan, Depression Plan, Pain Plan, Nutrition Plan (collectively, the “Services”), as selected and purchased by you.  BY REGISTERING ON THE WEBSITE WWW.MEDICALDNACLINIC.CA (THE “SITE”), USING THE SITE OR PURCHASING SERVICES OFFERED BY DNA MEDICAL CLINIC, YOU EXPRESSLY CONSENT TO SUCH SERVICES AND TO THE COLLECTION, use and disclosure of your personal information for the purposes described IN THIS PRIVACY POLICY AND as STATED IN ANY AGREEMENT OR DOCUMENT ENTERED INTO WITH DNA MEDICAL CLINIC.

DNA MEDICAL CLINIC considers the protection of personal information to be of fundamental value, particularly the protection of sensitive personal health, lifestyle and medical information. DNA MEDICAL CLINIC is concerned about privacy issues and wants you to be familiar with how we collect, use and disclose any personal information that individually identifies you (e.g. your name, address, phone number, e-mail address, date of birth, gender, age, etc).  Personal information means any information about an identifiable individual, but does not include the name or business contact information of an employee of an organization. How do we gather data by automatic means?

Sometimes we use cookie technology, web beacons and other similar devices on our Site to enhance functionality and navigation for our visitors. Information tracked through these mechanisms includes, but is not limited to: (i) your IP address; (ii) the type of web browser and operating system being used; (iii) the pages of the website a user visits; and (iv) other websites a user visited before visiting this website. Being able to access our cookies allows us to personalize this website and improve your experience at this website. How do we share personal information?

Except as described below, we do not sell, transfer or otherwise disclose to third parties the personal information we collect from you or on this Site.  At times, we provide your personal information to joint promotion partners and third parties who provide services on our behalf. These entities may use the information solely in conjunction with the specific promotion services they provide for us (e.g. fulfillment of an order; responding to your question or comment; providing a promotional prize; deployment of a consumer survey; investigation of a complaint). We may also use third-party service providers to assist us in aggregating and anonymizing your personal information, so that it does not include any information from which any individual can be identified.  We do not consider such aggregated and anonymized information to be personal information. You hereby expressly consent to DNA MEDICAL CLINIC using anonymous information that it has collected from you to prepare reports and other publications containing anonymous information only for internal use and for disclosure to third parties (including government, pharmaceutical companies, physicians, nutritionists, pharmacists, drug manufacturers and others involved in health care or in the medical field).

When DNA MEDICAL CLINIC genetic research staff or third-party collaborators or service providers publish results (on their websites or in academic journals) from tests and Services conducted by us, your genetic and self-reported information may be included within pooled summaries that are made public. Identification of your individual-level data from those summaries would be extremely difficult, but it is possible that a third party that has obtained some of your genetic data could compare that partial data to the published results and infer some of your survey responses.

We reserve the right to transfer any personal information we have about you in the event that we sell or transfer all or a portion of our business or assets to a third party. Further, we reserve the right to disclose any personal information as needed if such information is requested by law enforcement agencies or if we are requested to do so by law, regulation, court order, or by a government entity. We may also disclose your personal information to third parties if we have reason to believe that disclosing such information is necessary to: (i) conduct investigations of possible breaches of law; (ii) cooperate in any legal investigation; (iii) identify, contact, or bring legal action against someone who may be violating the Terms of Use of this website; or (iv) to protect our rights, privacy, safety or property.  Lastly, we may disclose your personal information for any other purpose to which you consent.

This website is intended for use by residents of Canada only. All matters relating to this website are governed by the laws of the Province of Ontario and the laws of Canada applicable therein. If you are located outside of Canada, and you contact us, please be advised that any personal information you provide to us will be transferred to Canada and that by submitting information, you explicitly authorize its transfer.

Some of these service providers may be located outside of Canada, including in the United States.
